 PRURIGO :

The term  prurigo refers to intensely itchy spots . It may be used when the cause is known or to describe a condition of unknown cause characterised by small itchy bumps .

Prurigo should be distinguished from pruritus , in which there are no primary skin lesionsrigo lesions . Prurigo has primary and secondary skin lesions . Primary prurigo lesion are dome -shaped papules . Secondary prurigo  lesions are scratched papules , scaly lesions , or thickened , darly pigmented areas . Scratching may lead to scarring . Prurigo presents as symmetrically distributed, small , intensely itchy , dome - shaped bumps . Sometimes there are tiny blisters . they are most numerous on the outer aspects of the limbs and buttocks , but may occur anywhere on head and neck , trunk and limbs . Prurigo may affect children  and adults .

Primary prurigo simplex include chronic prurigo of adults , prurigo mitis , and Hebra prurigo . When the spots are dark in color , it may be called prurigo pigmentosa . Primary prurigo papule is no longer visible because scratching has removed it . Prurigo may cause distress because severe itch can prevent sleep .

TREATMENTS OF PRURIGO :

Prurigo  is very hard to treat , but current therapies include steroids , dupixent , vitamins , thalidomide and UVB  light . In the event that staphylococcus  or other infection is present , antibiotics  have proven effective , but tend to cause more harm than good for this particular disease . A physician may administer a strong dose of  prednisone , cessation of steriods allows relapse to occur , usually within a few weeks. Another drug a physician may administer is Apo- Azathioprine , also known  by its brand name Imuran , is an immunosuppressive drug used in organ transplantation and autoimmune diseases and belongs to the chemicals class of purine analogues .

WHAT ARE THE CAUSES OF PRURIGO ?

The causes of prurigo nodularis is unknown , although other conditions may include PN. PN has been linked to Becker's nevus , linear disease , an autoimmune condition, liver disease and T cellls . Systemic pruritus has been linked to cholestasis , thyroid disease, polycythaemia rubra vera , uraemia , Hodgkins disease , HIV and other immunodeficieny diseases. Patients report an ongoing battle to distinguish themselves from those with psychiatric disorders ,  such as delusions of parasitosis and other psychiatric conditions .

SIGNS AND SYMPTOMS :

Nodules are discrete , generally symmetric , hyperpigmentation or purpuric and firm . They are greater  than 0.5 cm  in both width  and depth .They can appear on any part   of the body , but generally begin on the arms and legs .

Prurigo may appear to begin in the hair follicles

Prurigo pattern may be follicular .

In true prurigo noodularies  a nodule forms before itching begins in the hair follicles .

Excoriated lesions are often flat , umbilicated , or have  a crusted top.
